Scribe Mutual is the platform your doctor uses to take notes during your visit — and the place you go to read those notes when they're finished. Same day. No fax to your house. No records-request paperwork. Patient access is always free.
Getting access — your doctor invites you.
You can sign up for Scribe Mutual on your own, but the fastest way in is through an invitation from your doctor. Your doctor can send you a QR code to scan at the front desk, or a secure link texted to your phone. Tap it, set up your account, and you're in. The invitation code links your account with your medical records, so your chart is ready from day one.
If you sign up without an invitation, you'll have an account — but it won't be connected to your records until a clinician adds you to their practice.

Same-Day Notes — read what was written, the day of the visit.
When your doctor finishes the note from your visit and signs it, you can read it. That afternoon, that evening, by the time you get home — same day. No waiting weeks for a records department to mail something. No fax that never showed up. No portal queue that says "documents pending review."
This was the first thing we built, because it was the first thing patients told us they wanted. You sat in the room. You should be able to read what was written about the visit.
Notes appear in your account the moment your clinician signs them. You can read on your phone, on the web, or download a copy as a PDF if you want to keep your own files.
Stay in the Loop — know when something new arrives.
You get a notification when there's something new — a new note from a visit, an appointment coming up, or a record shared by another clinician on your care team. The notifications are direct: "Dr. Chen finalized the note from your June 10 visit."
You can turn notifications off, tune which ones come through, or just check in when you feel like it. The chart is there when you need it.
Send It Forward — share records with the people who need them.
Need to give your new specialist a copy of your most recent visit? Want to forward a discharge summary to a family member who's helping with your care? Scribe Mutual lets you share specific records to specific people through time-limited secure links — without anyone needing to call the records office.
The links are revocable. You decide what gets shared and for how long. The default share window is 72 hours, which is enough for the receiving office to download what they need and not so long that the link sits open forever.
If the person you're sharing with is also on Scribe Mutual — a co-managing clinician, for example — the record can flow directly to their view, without a link at all.
Your Care Team — everyone involved in your care, in one place.
As you see different clinicians — your primary care doctor, a specialist, a therapist — each one can be added to your care team inside Scribe Mutual. Your clinician adds themselves when they invite you, and any clinician you see afterward can join your care team with your consent.
Your care team view shows you who's involved in your care, when you last saw each of them, and what they've shared with you. If a new clinician joins your team, you'll know. If someone shares a note or record through the platform, it shows up under that clinician's name.
You stay in control of who's on your care team. If you stop seeing a provider or want to remove access, you can.
Family Records — for the people caring for someone else.
If you're caring for a parent, a child, or a spouse, you can link their records to your account through Premium and manage everything in one place. One sign-in. One notification stream. One view of who's been seen, when they're going back, and what was documented.
This is the part of the patient experience that matters most to people who aren't just managing their own care. We've kept it intentionally simple today, and we're listening carefully to caregivers in our beta to figure out what to build next.

What's coming next.
Some of the patient experience is intentionally small today, and a few features are still in development. Here's what we're working on next:
- Walk-In Link. Scan a QR code in a waiting room and you're connected to today's visit — no forms, no front-desk wait.
- Take It With You (Premium). Download your full history for offline reading, so your records travel with you even when you don't have signal.
- Smart Sharing (Premium). Share the right note with the right person based on where you are or where you're going — at the ER, at a specialist's office, at home with a caregiver.
We'll never enable a Premium feature on your account without telling you, and the free patient experience stays free for as long as Scribe Mutual exists.
Your records belong to you.
We built Scribe Mutual around the principle that medical records are yours.
- Access on your own initiative. You can view your records, your visit history, and your problem list any time, without a request form or a call to the office.
- Forward to whomever you choose. Specialists, family, caregivers, your next doctor — your call.
- Take them with you if you leave. If you ever stop using Scribe Mutual or change doctors, your records aren't trapped. We can export them in standard healthcare formats your next system speaks.
- Mutual audit trail. Every access to your records is logged — including ours — and the log is something both you and your clinician can see. Mutual visibility, not one-way oversight.
- We don't sell your data. Not your audio, not your notes, not the things that get logged about you.
